Q&A-44 antivirals to treat MS and AHSCT

Which antiviral would you recommend? Would you recommend I stop ocrelizumab and take another DMT? How does AHSCT work in relation to the EBV hypothesis?

Q1: Prof G, I have multiple sclerosis, and I am currently on ocrelizumab. However, I have smouldering MS with worsening fatigue and lower limb weakness. I am aware of several people with MS who report benefits from taking valacyclovir.  I am keen to start an antiviral. Which antiviral would you recommend? Would you recommend I stop ocrelizumab and take another DMT?

Q2: Most people with MS treated with AHSCT have EBV reactivation. If EBV causes MS, why are these people not getting worse? How does AHSCT work in relation to the EBV hypothesis?
