Q1: In your view, is primary progressive MS (PPMS) a smouldering disease from the start? Is it a pure smouldering MS?

Q2: Is the pathophysiology of PPMS the same or similar to non-relapsing secondary progressive MS?

Q3: Why is there a predilection towards spinal cord involvement in PPMS?

Much dogma concerning PPMS has crept into the field and become entrenched. There is a need to challenge the dogma, in particular, the assumption that (1) PPMS is non-inflammatory, (2) people with PPMS don’t have relapses, and (3) PPMS is a different disease to relapse-onset MS.
