My obstetrician is happy for me to have the infusion of ocrelizumab after delivery, but my neurologist has advised me to wait for 3-4 weeks before restarting ocrelizumab. What would you recommend?
I am a 21 year old man recently diagnosed with multiple sclerosis. I wanted to know if you can dedicate a case study directed toward young men with multiple sclerosis. There is not a lot of material out there specifically for young men with MS. I often see material directed towards women with MS on topics such as pregnancy. Meanwhile as a man with MS I wonder if I can even find love anymore. Thank you sir.
Just to reassure your patient - although I'm a bloke and have clearly never been pregnant - I had my last Ocrevus infusion in March 2021 and then switched to Mavenclad but had to wait a year. The switch was mainly because I want to try to reconstitute my immune system rather than remain immunosuppressed. We didn't start mavenclad for 12 months because my B cells were non-existent and my T cells were quite low as well. We only started because my neurologist got bored of waiting and even more bored of my nagging ;-)
Throughout that period there was almost no B cell reconstitution (I have a few now but not many) and I'm nearly 100kg so proportionally the Ocrevus was more diluted. I could probably have quite safely waited until summer of last year, as Prof G mentions, the B cell depletion is quite long lasting. My neuro didn't want to take the risk and that's fair enough. But really don't worry about waiting until the colostrum is cleared before starting back, it really shouldn't make any difference in terms of B cells (zero vs zero) and it will protect your baby from the steroids (!).

Is the effectiveness on lesion count as good with cladribine?
