Q&A 103 - Advanced MS - what can be done?

What type of MS do I have? Have I converted to having secondary progressive MS? What treatments are possible? Is there any possibility of switching to another DMT?

Question

I am on monthly ofatumumab (Kespimpta) injections, but as I am not mobile anymore, my consultant wants to stop my medication. My MS is getting worse despite no new lesions on my MRI scans over the last 4 years. I have lower limb weakness and spasticity, recurrent bladder infections and constipation. I have also noticed increasing weakness and clawing of my right hand. My fatigue levels have increased, and I now have problems concentrating. My memory is very poor. A significant factor in my recent life has been chronic stress and severe anxiety. I live alone with little family support and have recently had major financial stressors in my life.

What type of MS do I have? Have I converted to having secondary progressive MS?

What treatments are possible? Is there any possibility of switching to another DMT?
