Do all people with MS have smouldering disease? Do anti-CD20 therapies stop smouldering MS? I have smouldering MS on ocrelizumab; what should I do?

Q1: Prof G, do all people with MS have smouldering disease? And does early anti-CD20 therapy stop smouldering pathology? I have been on ocrelizumab for close to 4 years, and I am getting worse. My memory is worse, I can’t walk as far, I have sexual problems, and my fatigue is preventing me from working. What would you recommend I do?
